The Shape Theory NP operates as a self-pay telehealth medical practice.
By scheduling services or purchasing programs, you agree to the following financial terms.
1. Self-Pay Model
All services provided by The Shape Theory NP are self-pay. We do not bill insurance.
Patients are responsible for all consultation fees, medication costs, laboratory testing, and related services.
2. Consultation Fees
Consultation fees are due at the time of scheduling.
Consultation fees are non-refundable once the scheduled appointment time has occurred.
If an appointment is canceled at least 24 hours in advance, rescheduling may be offered at the provider’s discretion.
Missed appointments or late cancellations may forfeit the consultation fee.
3. Medication Purchases
Prescribed medications are dispensed through third-party licensed pharmacies.
Once a prescription has been:
- Processed
- Compounded
- Prepared
- Or shipped
Medication costs are non-refundable.
Due to state and federal regulations, dispensed medications cannot be returned.
4. Shipping & Handling
If medication shipping is selected:
- Cold chain handling fees may apply
- Overnight shipping costs are non-refundable once medication has been dispatched
Patients are responsible for providing accurate shipping information.
The Practice is not responsible for:
- Delivery delays caused by carriers
- Weather-related delays
- Incorrect address entry by the patient
5. Chargebacks & Payment Disputes
By purchasing services, you agree not to initiate unwarranted chargebacks or payment disputes for services rendered or medications dispensed.
In the event of a chargeback for legitimate services:
- Relevant documentation may be provided to the payment processor
- The Practice reserves the right to discontinue services
6. Laboratory Testing
If laboratory testing is required:
- Lab fees may be paid separately
- Lab services are non-refundable once ordered
Results are part of medical evaluation and not contingent upon treatment approval.
7. Program & Package Purchases (If Applicable)
If programs or multi-month packages are offered:
- Details of payment plans will be outlined at time of purchase
- Partial refunds for unused medical evaluations may be granted at provider discretion
- Medication costs remain non-refundable once dispensed
8. No Guarantee of Outcome
Payment for consultation or treatment does not guarantee:
- Eligibility for medication
- Specific medication approval
- Specific weight loss outcomes
Treatment decisions are based on medical appropriateness.
9. Policy Modifications
The Practice reserves the right to modify this Refund & Payment Policy at any time.
Updated versions will be posted with a revised Effective Date.
